Application & selection
Know what is required before you begin.
The application is designed for eligible UT Tyler student pharmacists who want sustained training within an integrated health system and are prepared for a longitudinal experience.
Application materials
- Completed online application
- Current curriculum vitae
- Applicant-authored letter of intent or personal statement
- Two confidential professional reference evaluations
- FCOP standing verification, as required
Letter of intent
Use a professional 1–2 page format. Address career goals, interest in longitudinal learning, leadership or service, strengths, meaningful growth areas, and program fit.
Disclose material AI assistance consistently with FCOP policy.
Professional references
Choose two evaluators who can speak specifically about your professionalism, communication, accountability, initiative, teamwork, dependability, and readiness. References submit confidential evaluations through the separate pathway.
Selection process
Review may include the application, CV and experience, references, academic and professional standing, leadership and service, and a structured interview. Participation remains subject to FCOP and institutional requirements.
